TACE - Brain Injury Program
TACE provides case management services to prevent the unnecessary institutionalization of disabled individuals who have suffered a brain injury. The case management program, TACE, provides supports and assistance to the individuals who have suffered a brain injury to continue to live in the community.
The involvement of TACE with individuals "At Risk" of unnecessary institutionalization is due to our work as case managers in the community.
Funded by the Illinois Department of Human Services, Office of Rehabilitation Services, Home Services program since 2003, TACE provides Brain Injury Case Management program services in the Chicago Metropolitan area.
